چکیده

ENDOCARDIAL FIBROSIS occurs in diverse diseases. An infantile form appears to reflect an unexplained congenital derangement in which a thick layer of collagen and elastin lines the left ventricular cavity.' Sporadic instances noted in the adolescent have also been presumed to be of congenital origin.2 A poorly understood cause of heart failure occurs frequently in Uganda and South Africa in association with extensive endocardial fibrosis.3 Endocardial fibrosis also occurs as one of the pathological changes of subendocardial myocardial infarction, and, in the right ventricle, in the carcinoid syndrome. Localized areas of endocardial fibrosis have been noted in association with sustained hypertension and with valvular stenosis and regurgitation.4 Despite the frequent occurrence of this abnormality in many types of heart disease, proof of endocardial fibrosis has not been possible except

A Simultaneous X-MRI and Non Contact Mapping Study of the Acute Hemodynamic Effect of Left Ventricular Endocardial and Epicardial Cardiac Resynchronization Therapy in Humans Running Title: Ginks et al:Mechanistic Insights Using Endocardial Pacing In Humans

Background Cardiac resynchronization therapy (CRT) utilising endocardial left ventricular (LV) pacing may be superior to conventional CRT. We studied the acute hemodynamic response to conventional CRT and LV pacing from different endocardial sites using a combined cardiac MRI and LV non-contact mapping protocol to gain insights into the underlying mechanisms. Methods and Results Fifteen patient...

A simultaneous X-Ray/MRI and noncontact mapping study of the acute hemodynamic effect of left ventricular endocardial and epicardial cardiac resynchronization therapy in humans.

BACKGROUND Cardiac resynchronization therapy (CRT) using endocardial left ventricular (LV) pacing may be superior to conventional CRT. We studied the acute hemodynamic response to conventional CRT and LV pacing from different endocardial sites using a combined cardiac MRI and LV noncontact mapping (NCM) protocol to gain insights into the underlying mechanisms. METHODS AND RESULTS Fifteen pati...

Left ventricular endocardial or triventricular pacing to optimize cardiac resynchronization therapy in a chronic canine model of ischemic heart failure.

Cardiac resynchronization therapy (CRT) is a proven treatment for heart failure but ~30% of patients appear to not benefit from the therapy. Left ventricular (LV) endocardial and multisite epicardial [triventricular (TriV)] pacing have been proposed as alternatives to traditional LV transvenous epicardial pacing, but no study has directly compared the hemodynamic effects of these approaches. Le...

Targeted left ventricular endocardial pacing using a steerable introducing guide catheter and active fixation pacing lead.

AIMS Cardiac resynchronization therapy via the coronary sinus (CS) is not always possible. Left ventricular (LV) endocardial lead placement is a potential alternative. The purpose of this study was to assess the feasibility of endocardial LV pacing using a steerable lead introducer and active fixation polyurethane lead. METHODS AND RESULTS Endocardial LV lead placement was attempted in nine p...

Haemodynamic evaluation of alternative left ventricular endocardial pacing sites in clinical non-responders to cardiac resynchronisation therapy

INTRODUCTION Non response to cardiac resynchronisation therapy (CRT) may be related to the position of the coronary sinus lead. METHODS We studied the acute haemodynamic response (AHR) from alternative left ventricular (LV) endocardial pacing sites in clinical non-responders to CRT. AHR and the interval from QRS onset to LV sensing (Q-LV interval) from four different endocardial pacing sites ...
